This questionnaire has been designed to ensure that we get the most of our time together. So please provide as much information as possible. If you are not comfortable answering any of the questions please leave them blank.

Consultations
Where do you want the consultation to take place? *
In Person consults can be held at your home or in a mutually convenient place (Coffee Shop, Library, Park, etc). Please add £5 for consults held in locations over 15 minutes drive away from ME16 8EA to allow for transport costs

If you would like a consult in your home, please give the postcode so I can know how much travel time is needed. All locations further than 15 minutes from ME16 8EA will have an extra charge depending on how far away it is, unless otherwise agreed in advance.

What do you hope to gain from our consult? *
For example - Feeding in slings, back carrying, help getting it right/more comfortable, tandem carrying, etc. Please include any carriers you have already used/own.

Consultations must be paid for in advance. Your slot is not secured and may be allocated to someone else if it is left unpaid.

If you need to cancel or rearrange your consultation slot, please let me know as soon as possible. Any cancellations within 24 hours of the start time will be charged a 50% cancellation fee. If you are not present for your appointment I will not be able to refund or transfer your booking.
