Please fill out this application form, regardless of whether you are volunteering to be a new Shreiber Family Pet Therapy Volunteer at Rowan University, or if you have volunteered in the last under auspices of another event or organization. Please fill out an application for each dog-provider pair. For example if you are certified with 2 different dogs, you would need to fill out 2 different applications. And, if you and your spouse are both certified with your dog, you would each need to fill out your own application.
