This form is to apply for 1-hour of CTLE/Certificates of Participation for any NYS TESOL webinar/event you have participated in.  Please fill out all the fields as needed below and allow a few weeks for processing.  You will need to complete a new form for each webinar you wish to apply for CTLE/Certificates of Presentation/Attendance. Only members can apply for CTLE or Certificate of Participation.
