1.在本会员申请表中, “你”指会员申请者, “我们”指杨琳健身舞蹈学校(YL Group Inc). 请仔细阅读所有条款.该合同属于安省消费者权益保护法范围.所有在该申请表内的免责或限制你的权利只适用于消费者保护法允许的范围.In this agreement (The Agreement), the member is referred to as “you” and YL Group Inc is referred to as “us” or “YL Group”. Please read everything carefully. This contract is subject to the Consumer Protection Act (Ontario) (the “Act”). Any waiver or limitation of your rights in this agreement applies only to the extent permitted by the Act.

2.我们的责任：通过签署该申请表,我们接受你作为我的会员.在成为会员期间,你可以在我们营业时间内,参加你够买的我们提供的健身课程.Our responsibility: By signing this agreement we accept you as a member of our school. You may attend the classes that you purchased during the term of this agreement, according to our schedules.

3.你的责任：通过签署该申请表,你同意：a.支付所购买课程的全部费用 b.所用该申请表涉及的术语和条款 c.遵守我们的规章制度和管理  d. 如果电话、住址和其它联系方式发生改变时,及时通知我们.Your responsibility: By signing this agreement you agree: a). to make payment in full for the class you purchased; b) to observe all terms and conditions which are set out in this agreement, c). to follow our rules and regulations(“Rules”) d). to immediately notify us of contact information change, such as phone number, mailing address, etc.

4.你允许我们使用你在参与我们的各项活动中,拍摄的照片、视频等,作为教学、宣传用.You allow us to use the photos, videos, etc. you take during your participation in our various activities for teaching and publicity purposes.

5.会员有效期：a)次卡：从交费起半年内有效 b)续卡：从交续费起,到会员有效期结束.Duration of membership: (a) Class member (ten classes): six month since registration; (b) extended membership: from the date of payment till the expiration date.

6.确认：你,会员,确认被告知在参加YL健身舞蹈及其他活动中可能会有风险.你是自愿参与并且知道自己面对的可能的风险和伤害,包括所有疾病和损伤.如果你有任何担忧,请在开始参加我们的课程前与你的医生确认.你同意放弃在活动中由于你自己的行为(包括你的疏忽) 造成的损失和伤害 (已知或未知),而对我们,包括杨琳健身舞蹈的委托人(如果有),相关的业主,办公人员,主管,代理,雇员,志愿者或合同工的进行赔偿和诉求.我们不负责任何个人财产的损坏、丢失或被盗.Release: You, the member, acknowledge that there is a risk associated with participation in fitness activities and exercising. Your participation is completely voluntary and by using the facility you acknowledge that you are assuming all risks and injury to yourself or others including any illness and medical condition. Please raise any concerns about starting a fitness program or exercise with your physician before attending our class. You agree on your own behalf (and behalf of your personal representative, heirs, estate trustee or assigns) to a) release indemnify and discharge YL Group Inc and the Franchisor (where applicable), including the respective owners, officers, directors, agents, employees, volunteers or independent contractors from any and all claims or causes of action(known or unknown) brought against us by any party arising out of your actions, including your negligence, while at the facility. We, YL Group, are not responsible for any damage to, loss or theft of personal property.

7.办会员卡后概不退换 No refund for membership.
