Thank you for participating in Recycling Champions!

To submit:

1. Use this form to mark the Recycling Champions tasks you have completed. Only ONE submission per HOUSEHOLD.

2. Email your photos to admin@cooloc.org with the subject title "Name-Address-Recycling Champions" by December 15, 2022 (extended to February 28, 2023).

NOTE: If you have completed a task more than once (for example, replace single-use products with reusable alternatives), please include all relevant photos in your email.
