Dining Out For Life® Participant Agreement 2020
As a participant, this business agrees to participate in Dining Out For Life on Thursday, April 30, 2020, and to send or deliver a check for the total contribution payable to Project Angel Heart no later than May 31, 2020.

Contribute 25% of gross food and/or alcohol sales made on April 30, 2020, to Project Angel Heart, regardless of any other promotions that day. Minimum contribution of $300 if percentage of sales is less than $300. *
Promote Dining Out For Life® by: displaying and/or distributing event materials a minimum of 3 weeks before the event and informing wait staff, managers, and other personnel about Dining Out For Life so they can promote the event to diners and answer basic event questions.
