In keeping with the City of Ottawa health requirements, the GSH Swim Club is required to verify that all of its parents meet the vaccination requirements prior to entry into CRC or RRC.

To comply with this requirement we are asking parents to:

1) show (a schedule will be set up) or email (to secretary@swimgsh.ca) proof of vaccination.  
We also require consent that we record and store your vaccination status until the city no longer requires it.
 Your information will never be shared and will be discarded as soon as this protocol is no longer required.

2) You must also fill out the Parent Self Screen every time you enter a facility (found on the GSH Web Page) https://docs.google.com/forms/d/e/1FAIpQLScv-QcMMUMtE6SYRERTOYlYZFxJul8KcFrgG8QVg8hIaLlWvQ/viewform

-Access is for the facility lobby only, parents are not permitted on the pool deck during GSH swim practices.
-If you are not planning to enter the facility you do not need to fill out this form or show proof of vaccination
