CONFIDENTIALITY STATEMENT
I understand that my access to student academic records is limited to my need to know for the purpose of performing my duties as a Clinical Educator for a UNC Charlotte Teacher Education candidate. I will not release to any other party information contained in the records, except in accordance with University Policy Statement #69 (http://www.legal.uncc.edu/policies/ps-69.html). I will not discuss or relay information contained within any record to any one who is not authorized by the Dean or Associate Dean of the College of Education. When discussing confidential information with other UNC Charlotte employees in the course of my workday, I will exercise care to keep the conversation private and make sure I am not overheard by others who do not have access to or a need to know the confidential information. I will also maintain my password in confidence I will not disclose it to any other person or authorize others to use it. I further understand that any violation of this agreement will result in my being removed from the Clinical Educator role, and that I will no longer be eligible to host UNC Charlotte Teacher Education candidates. I acknowledge that any disclosure of confidential information by me to any unauthorized person could subject me to criminal and civil penalties imposed by law. I understand that my obligation to keep the records confidential shall remain in place in perpetuity, including after my role as a Clinical Educator has ended, and hereby agree to indemnify and hold harmless UNC Charlotte and its agents for all claims from my failure to comply with this requirement.
