(i) I hereby confirm my willingness to return to India in accordance with the terms of the Government of India/Embassy of India, as may be prescribed from time to time.

(ii) I confirm my readiness to follow all instructions given by the officials of Government of India/Embassy of India/Aircraft Crew/Medical Personnel on arrival.

(iii) I agree to bear the applicable airfare that will be charged by the airline concerned.

(iv) I agree to undergo a 14-days mandatory quarantine on my arrival in India at my own expense as per the protocols framed by the Government of India.

(v) I will sign an Undertaking and submit 3 copies of the same before departure from the host Country, to concerned authorities in the Government of India.

(vi) I will be making this journey at my own risk.


NOTE:

(i) Incomplete forms will not be considered

(ii) Mere filling of this form does not automatically grant the privilege of repatriation to India. Decision solely rests with the Government of India.

(iii) On successful submission of the form, a pop up stating “Your form has been successfully submitted. No separate e-mail shall be sent" appears. There is no requirement of sending any further email to the Embassy of India in this regard.

(iv) Kindly fill the form accurately and carefully the first time itself. Please check all details thoroughly before pressing “Submit”. Submission of multiple forms will create duplication, which should be avoided.
