I affirm that my child is physically capable of performing the activity level required during training, practices, tryouts, workouts and games. Furthermore, I will not hold the coaches, assisting coaches, any team members of the Georgia Raptors, or any facility/field at which training, practice, or game competition takes place, liable for any injury or loss of property sustained while a player or trying out to be a player of the Georgia Raptors. As such, I consent to my child participating in all scheduled team activities. Agreeing to this affirms my consent for my child to participate in all activities and events required to be a player with the Georgia Raptors. *
