Students wishing to bring an emotional support animal, also known as a comfort animal or assistance animal, to a residence hall or campus apartment must complete the following:
- Submit this form to register your animal
- Provide documentation
from a qualified medical provider explaining why the animal is needed
- Send up-to-date vaccination records (where appropriate).
