2022 All District Band Audition Director Registration Form - Online Registration Deadline December 8, 2021

Please be sure to read the Registration Instructions sent to the email list on 11/22/21.

Directors must complete a separate registration for middle school and high school.

Directors will be expected to mail the registration fee and confirmation email (for both director registration and student registration forms) to the appropriate chair postmarked by Friday, December 13, 2021. A separate check will be due for each school (middle/high) that students are registered under. Students who audition on 2 instruments need to be entered as 2 separate student entries and will have two audition fees. Fees are $10 flat fee per school + $8 per audition. You will receive a bill within 12 hours of completing registration.

A copy of this form will be emailed to the director's school email address as entered below as verification that the online portion of the registration process is complete.
