1. 體社聯團體會員申請費為港幣1,000.00元正，如申請獲得接納，需按繳費通知繳交申請費用；The membership application fee of FHKSO is HK$1,000.00;Application fee should be paid according to the payment notice if the application is accepted;

2. 負責人必須同時附上:商業登記證/社團註冊證明書副本及團體業務地址證明副本；Responsible Person must submit copies of both Business Registration Certificate/Certificate of Registration of a Society and organisational business address certificate;

3.負責人必須年滿十八歲並持有有效香港身份證，副本需連同本表一併遞交；Responsible Person must be aged at 18 or above with a valid Hong Kong Identity Card. A copy of the identity card must be enclosed with this application form;

4.填妥本表，連同備註(2)﹑(3)文件及申請費用，親身遞交或郵寄至體社聯辦事處(香港灣仔軒尼詩道393-407號東區商業大廈904室)。Submit  the  completed form  with all documents  stated on  Remarks (2), (3)and application fee in person or by  post to the office  of “Federation  of  Hong  Kong  Sports  Organisations  Limited”  (Room  904., Eastern  Commercial  Centre, 393-407Hennessy Road, Wan Chai, Hong Kong).
