Visitor Administration Approval Form (To be completed by the Guest's Administrator)
TO BE COMPLETED BY AN ADMINISTRATOR AT THE GUEST’S HIGH SCHOOL.
1) The student (MHS or Guest) emails this link to the Guest's High School Administration (Principal, AP, AD, or Dean)
2) The visiting student's administrator will complete this form
3) The guest must be a current 11th or 12th grade student in good standing
4) Mr. Schultz will confirm this form when completed and email the administrator. Any invalid email address will lead to both students NOT attending with NO refund.
