Caregiver Declaration (for rangatahi/tamariki):
By completing this form, I give permission for the tauira named above, who is 10 years old or older, to participate in this kaupapa.
All attendees must be enrolled to participate.
Tamariki/participants may arrive 15 mins prior to the beginning of the kaupapa and MUST be signed in by a parent or caregiver.
If your tamariki is travelling to the Māoriland Hub independently, you must email Louisa Donnell louisa@maorilandfilm.co.nz to let our team know their arrival time.
Māoriland is responsible for your tamariki whilst they are inside the MHub. If we don’t know that they are coming (independently) we cannot alert you if they fail to show up.
After the kaupapa finishes, your tamariki MUST be picked up from inside the Māoriland Hub. If you are sending someone else to pick them up please text Lou, 0224253026. We will keep them inside the whare as we don’t want tamariki waiting by themselves by the road.

Any changes to the schedule will be communicated by text and email a minimum of 24 hours ahead of the kaupapa
