This form is to gather information from parents requesting School Transportation for their child(ren).

1. Buses are assigned on a first come first serve basis.  

2. ALL parents are REQUIRED to submit this form to receive bus service.

3. Applications from students who have received Face-to-Face instruction during the March 22, 2021 session will be considered First.

4. Approval of all Transportation requests will depend solely upon seating capacity availability.
