2024 Midwest Bible Campout Class Registration

Please complete one registration form per family. This information is used to help our teachers prepare enough class materials for all the children. Register children from preschool age through 5th grade. The Junior High class includes 6th, 7th and 8th grades.

Preschool (Ages 3 and 4)

List the names and ages of children that will be going into preschool in the fall.

Kindergarten & First Grade

List the names of children that will be going into kindergarten or first grade in the fall.

Second & Third Grade

List the names of children that will be going into second or third grade in the fall.

Fourth & Fifth Grade

List the names of children that will be going into fourth or fifth grade in the fall.

Responsible Adult

Each child must have a responsible adult that is also attending the Midwest Bible Campout
