本表格只供參與香港同志遊行2019之非牟利團體、社團、小組或商業機構代表參加者使用。
團體之定義為必須符合以下條件：
一） 由同志及跨性別團體之個人或親友組成；或
二） 由同志及跨性別團體組成；或
三） 由非牟利團體組成，該團體向以支持同志及跨性別社群，並期望藉著參與香港同志遊行以表達對社群之支持；或
四） 由同志及跨性別人士營運或擁有之商業機構；或支持同志及跨性別社群之商業機構。
註：不可在遊行前或遊行期間售賣商品。

報名團體的名字會刊登在大會場刊中，如未能於大會仍會盡量安排隊伍參與，但團體名字將不會刊登於場刊中。

This form is only for Non-profit making organizations, community groups or representatives of corporations.
The definition of Groups must comply with the following regulations:
1) Formed by LGBT individuals or their families and friends, or
2) Formed by LGBT organizations, or
3) Formed by non-profit organizations who have always shown support to the LGBT community and wish to participate in the Parade to show their support, or
4) Corporations that are operated or owned by LGBT or who support LGBT.
*No selling of products is allowed before and during the Parade
