  1. The submission of this form will be recorded as official notice of withdrawal. The date this form is submitted will constitute the official date of notice regarding camp withdrawal.
  2. Minimum of one week's notice is required.
  3. All refunds are subject to a 15% administrative fee.
  4. Alternatively, you may choose to place your registration fees "on account" with us (no administrative fee charged) to be used in the future.
