Please use this form to indicate that you have conducted a screening for your child (including volunteers) otherwise your child would not be attending class in person today (school day). Adult students, teachers and staff members must complete the below screening prior to coming to campus as well.

The form must be completed on Sundays (school day) by 11:30 AM, even if your child or yourself (for adult students, teachers and staff) will be absent. Please contact the teacher if the student will be absent from school.
