Commemorative Paver Order Form
The Grafton School District's Enhancing Our Future athletic complex development project continues to move along, with Phases 1 and 2 completed and fundraising to complete the entire project well underway! You can support the project and leave a lasting legacy by purchasing a custom commemorative paver that will be laid in the entry plaza at Kellner Field. Three different sizes of pavers are available.  Pavers will be customized with your personalized message to honor a loved one, share a memory, make a tribute, or show your support for the Black Hawks!

To order more than one paver, please submit a separate form for each one.

Payment (cash or check only) should be mailed to or dropped off at the Grafton School District Office, 1900 Washington St., Grafton, WI   53024.
