Please complete and submit this form for each official HIGH SCHOOL TRANSCRIPT that you are requesting to be sent.

Your Universities will need official transcripts from any colleges you have attended in order to transfer your college credits to their institution. You will need to order official transcripts directly from CCC, COCC, PCC, MHCC, etc.. You can order your college transcripts through your college account. (myclackamas, mypcc, mymhcc, etc.)

This form is set to send you a copy of its submission. If you do not receive a copy of your answers, it means that your form was not successfully submitted and that you will need to re-submit the form.
