Thank you for your interest in the 1st Annual Save-A-Seed Juneteenth Kickball Tournament! After completing the form in full, the registration fee of $40 per team that must be paid before you are considered registered.

Teams must have between 8 to 10 people and each person's name must be listed on this registration form in order to play. ***THE CAPTAIN OF THE TEAM WILL BE RESPONSIBLE FOR ALL CORRESPONDENCE WITH YOUR TEAM.***
