*Please only complete this form if your child is new to Paris Schools*
Complete this form to register your student for the 2023-2024 School Year.
If you are enrolling multiple students, please submit one form per student.
Once registration is complete, your student(s) will be enrolled into Infinite Campus (our student information system). If you are Out of District then you will need to contact the school principal to set up a meeting. All Out of District Request must be approved by the Principal.

Required Documents for Registration:
Student's Birth Certificate
Up-to-date KY Immunization Certificate
Proof of Residence, such as utility bill, tax bill/tax receipt, lease agreement, property deed, sale agreement.
Other Registration Documents that will need to be sent prior to August 9, 2023:
Social Security Card
Court Documents
Proof of a preventative health care exam
Proof of an eye exam (After January 1, 2024)
Proof of a dental exam (After January 1, 2024)
