In order to request information be released from ODR, this form should be completed in its entirety. ODR will then send a Docusign Release of Information Authorization to the email account provided for completion. Docusign is where you will indicate information to be released and release details.

If your last date of attendance at App State is more than five years ago, ODR will no longer have records.

Confirmation will be sent when materials are released.
