To help us keep track of our raffle items for our annual conference, which many times may come from various places, and arrive at differing times and again at a multitude of locations, we are asking that you fill out this form for those items.   This will give us a central list that we can return to after the conference and review, and make sure that we are able to equally recognize everyone.
