Thank you for your interest in announcing your student's birthday on the CBE Marquee.  The PTO appreciates your support!  Due to our large student population, it is impossible to guarantee that your student's birthday will appear on the exact date requested; however, we will make EVERY effort to accommodate your requests.  The announcement will be posted for one week.   The fundraising fee is $25 per child and must be paid at the time the form is submitted.  ALL REQUESTS MUST BE SUBMITTED 30 DAYS PRIOR TO THE STUDENT'S BIRTHDAY AND MUST BE ON THIS FORM.  Please submit one form for each date requested.
