I understand that the parents/legal guardians are fully responsible for monitoring the health of the child and family members of the student before checking into Kid City.   By checking into Kid City today, I am confirming that no one in my family has been sick for the past 48 hours with flu like symptoms/fever.   I understand that social distancing children is not possible.  I understand that there is always a risk to catch illnesses such as, but not limited to influenza, MRSA, COVID-19.   I (parent/legal guardian) assume the risks involved and we release, waive, discharge and hold harmless First Baptist Church and their volunteers from any and all liability, claims, lawsuits, expenses, demands, and cause of actions while participating in any activity while in, on, or around the premises of First Baptist Church of Tempe.
