This form is to be filled out for people wishing to join the Quarry Park Disc Golf Club. Note: this is a separate membership to the Greens Fees Membership that the course offers. To play at the course, Club members will still have to pay current greens fees.

- Participate in Club Vs Club rounds.
- Access to the Members Only Quarry Park Disc Golf Club Facebook Page.
- Other Member Benefits to be announced during the season.
Payment options is bank transfer (Quarry Park Disc Golf Club, 30-99-50, 44697860). Please put "initials 24 Memb." or similar in the description. Some banks limit the characters so whatever you can fit in!
