Three Questions for Transitioners

Thanks for taking a moment to look in at these questions. Your answers will feed into a process to develop a Transition Hub for England and Wales.

There are two ways to answer these questions:

- One is on behalf of a Transition Group. Please take these 3 questions to your group for discussion, and simply fill in the form on behalf of the group afterwards. Please just indicate that you are responding on behalf of a group to be taken to the right section of the form.

- The other is as yourself. Again, please just indicate that you are responding as an individual to be taken to the right section of the form.
