ESSER III Patron Data Collection Form
The American Rescue Plan Act of 2021 includes nearly $122 billion for Elementary and Secondary School Emergency Relief (ESSER III).  These funds are to be used to take additional steps towards continued safe in-person learning in schools.  This form will be used to collect data on the importance of how ESSER III Funds are distributed amongst district initiatives.

Creek Valley Schools would like to use ESSER III funds to completely update the high school with a new HVAC system, which will allow for cleaner air in all areas of the building.  Do you feel, in our time of COVID, that fresh, outdoor, filtered air will be a benefit our district? *
Creek Valley Schools would also like to use ESSER III funds to help with loss of learning, due to mental and behavioral needs of students.  To accomplish this, we have hired an elementary counselor trained in mental and behavioral counseling.  Do you feel this would be a benefit to our district? *
