Please complete the following form for each student who will need transportation for the 22-23 School Year. Please note, students in 7th and 8th grade will receive an annual SEPTA Transpass. 7th/8th grade students still require a request from a parent/guardian to receive a SEPTA Transpass. The district no longer provides SEPTA 2 Trip passes.

If you have recently moved, ensure you have submitted proof of residency to the Main Office, either in person or via email, to have transportation accurately assigned for your student.

A reminder: This is just a request, it does not guarantee your student will be assigned yellow school bus transportation. Your child(ren) may be assigned a SEPTA Transpass as an alternative option. This decision is up to the School District of Philadelphia.
