The following information is included in the Church's Sacramental Record. Why do we need this? All Roman Catholic Sacramental records are collected at the Church where an individual's Baptism was recorded. We require the correct date and address of your child's baptism, so that when he or she receives any sacrament, we will send the documentation to that church.

The Program Fee is $50 for First Reconciliation Prep and $50 for First Eucharist Prep (totaling $100 for both sacraments). This fee pays for educational materials, workshop supplies, and craft materials. *
