When: Thursday, March 20th - 12:30-4:30 (depending on traffic)

Cost: $30 per student (billed to Procare), students may bring money from home if they would like to play at the arcade.

Medical Release:  I authorize Underwood After School Program staff who are trained in the basics of first aid/CPR to give my child first aid/CPR when appropriate..  I understand that every effort will be made to contact me in the event of an emergency requiring medical attention for my child. However, if I cannot be reached, I hereby authorize the program to transport my child to an emergency facility as necessary to secure medical treatment for my child. I/we hereby release and discharge UASP from any and all claims which may arise out of the decision to provide emergency medical care. I understand that this field trip may expose my child to some risks and I assume any such risk that may arise there from. I accept full responsibility for all medical expenses for any injuries that might occur to my child by reason of his/her participation
