This form is for the intermediate and middle school campuses.  If your child does not attend MIS or HPMS campus, you have selected the wrong form.  Go back to the main support page and select the correct campus form. Also remember:

-  be sure you are using the Chrome browser, even on a Mac computer and your student has logged into Chrome.
