Florida Academy of Collaborative Professionals Practice Survey For Professionals
This survey is for Collaborative Professionals in Florida to fill out upon completion or termination of a Collaborative Family Law matter.  The purpose of this survey is to gather and analyze data about how the Collaborative Process is being practiced in the state and add a valuable debriefing tool to professionals’ toolboxes.

For purposes of this survey, a Collaborative matter is a matter where, at a minimum, (i) each client had an attorney, and (ii) the clients signed a Participation Agreement that (iii) contained a disqualification clause prohibiting the attorneys from engaging in litigation on behalf of the clients.  A matter does not become “Collaborative” until all clients have signed a collaborative participation agreement.

Part A requests statistical information, and Part B requests subjective information about the professionals’ experiences and client dynamics.  

THE TEAM SHOULD COMPLETE ONE SURVEY AT THE CONCLUSION OF EACH MATTER, REGARDLESS OF RESULT.
ONLY ONE (1) SURVEY FORM SHOULD BE SUBMITTED FOR EACH MATTER.  

By completing this form, you certify that you are the professional in your matter designated to complete this survey.

Prior to filling out the survey, professionals should have the following information readily available:

1) The date the participation agreement was signed;
2) The amount of time each professional contributed to the matter
3) The total fees charged by each professional
4) The total amount of hours you met with the clients individually
5) The total amount of full Team meetings held


This survey should take about 15 minutes to complete, though we encourage the professionals to engage in additional discussion to assess their collaborative matter.
