WORKBOOK: Student Workbook must be picked up at 440 Lenox Road, #1J (call 718-270-2983) to arrange pickup (bring a copy of your receipt)
NOTE: Registration is a 2 step process. After filling out this form, you will need to make payment for the course in order to secure your seat.
