Due to limited space in Didsbury Mosque, following the government COVID-19 guidance for safe use of places of worship, Friday prayers will be only attended by using this booking form to apply for a place inside the mosque.
DO NOT USE THIS FORM:
• If you have any COVID-19 symptoms (temp. over 37.8c, a dry cough, loss of smell or taste, cold or flu like
symptoms).
• If you are under 12 years old.
• If you or one of your household arrived to UK in last 14 days.

Children under 12 will not be allowed in the masjid, due to difficulty in keeping social distancing
