Willard Elementary Parent Request Form 2023-24

This request form provides two sections for parents/guardians to share request and additional information to be considered as class placements are determined for the 2023-2024 school year. Information on this form, along with other academic and school information will be used to create classes that are balanced according to range of student ability, behaviors, gender, and the number of students. Please follow the guidelines found in each section. You are welcome to place information in any or all of the sections.
