By filling out this form, your organization could be selected to receive a waiver covering this year's dPlan|ArtsReady subscription cost of $59.40. These fee waivers are intended to benefit arts organizations that provide performing arts programming to the public and that wish to use dPlan|ArtsReady, however, this waiver does not apply to current dPlan users.

Any nonprofit performing arts organization located in the United States, District of Columbia, and US Territories that wishes to request a one-year free subscription to dPlan|ArtsReady—the updated, intuitive online emergency preparedness tool—must complete and submit their information on the form below.
