Please refer to the terms and conditions* of your pass/plan for cancellation advance notice periods (30 days notice for the majority of our memberships) and any early cancellation fees. In order to modify or cancel a membership, you must notify FIT4MOM Rocket City via this form. Please note instructors are not authorized to cancel or modify memberships.

A confirmation email acknowledging receipt of this form will be emailed to you within 4-5 business days. Your advance notice period will start from the day the form is submitted.
