Please submit this form within 24 hours of the date of the student’s absence.

Illness Related Absences:

  • If longer than 3 consecutive days, email a doctor's note to attendanceinfo@hsd294.us

Medical Appointments:

  • Email a doctor’s note to attendanceinfo@hsd294.us  with the date and time of the appointment.

Vacations:
  • Need advanced approval
  • After approval -  excused absences will appear in Infinite Campus.
  • Before vacation - get make-up work from teacher(s).
Computer Problems: 
Power and Internet Outages: 
  • These are not excused absences. 
  • Have a backup plan in place if this happens (libraries, coffee shops, or relative’s house)

This form may only be filled out by a legal guardian or student 18 and over. Falsely reporting absences may be considered an act of academic fraud, which is subject to disciplinary action under the guidelines found in the MNVA Handbook.
