By filling out this survey, you are requesting membership in the OVCA High School Speech and Drama club. Membership of this club entails participation by attending bi-weekly meetings and engaging in some activities and performances, and possible optional outings to which I would need to travel. Students will learn about and engage in activities such as:
--Improv
--Characterization
--Set, costume, prop design
--Expression of self
--Speech presentation skills
--Team building
--Performance
--And other theatre-related activities

 Drama and speech related activities and performance pieces are meant to teach and challenge students in the art of drama and speech. At times, drama/speech selections and activities may touch on content and themes that are mature and/or controversial. In any activity, we hope to challenge our pursuit of the art and perfect our performances through a variety of muses, including real life. Our Speech and Drama Club will be a safe place of mutual respect, tolerance, team-building, and critique of performance. After filling out this survey, your Learning Coach will be notified and be asked to give their written permission for you to join the club.

I understand that violation of this clause will revoke my membership and could entail consequences that involve school administration, depending on the degree of offence. *
I understand that my participation in Speech and Drama Club is purely extra-curricular and will not replace or hinder any academic requirements for my schooling. *
I understand that there is a possibility of performance and/or activities that require me to travel (ex: putting on a performance on location or attending plays/performances). If and when those arise, I will need to obtain my own means of transportation and my parent/legal guardian will need give their permission for that particular activity. *
I understand that if I agree to take part and perform in a group performance, that I become an integral part of that activity on which my cast members and directors rely, and I will attend all meetings, rehearsals, and performances pursuant of that part. If I fail to be present for activities that I have signed up for without previous approval for extreme circumstances excusing my absence, then I will be resending my part and putting my membership in jeopardy. *
