Supplies Request Form
Complete this form in its entirety if you are interested in receiving nonperishable food items; hygiene products and school supplies.

Items will be available for pick up every 1st and 3rd Monday between the hours of (8am-9am & 1pm- 2pm) while supplies last.

Families may receive one distribution per month per child enrolled at AWHS.  This is contingent upon available supplies.
