SACK LUNCH FOR FIELD TRIPS REQUEST 2023-2024 SY
Please complete the following form which will then be shared with your building principal, building secretary, and your building head cook (Toni Fassig at all three schools).
Please remember - the cafeteria is requesting ONE FORM PER TRIP with all the students who need meals.  Teachers, please work together to send in ONE REQUEST FORM with one adult responsible for the cold lunches.

All sack lunches must meet the federal government components for a reimbursable meal and must not be unpacked in the school or otherwise tampered with in regards to content.  If there are special circumstances that need to be addressed (ie- bus company does not allow drinks on the bus, etc.) the kitchen staff must know this ahead of time so that the situation can be handled in the best possible way in regards to meeting the district's responsibilities as participants in the National School Lunch Program.  Please make notes in comment area below.

Name of person who will be picking up the sack lunches? An adult (teacher or other chaperones name) - should pick up the box of meals at one time.  Please remember,  you must return a completed roster to the office prior to the trip leaving so that all meals can be properly charged to accounts as required by the NSLP guidelines. You can print a copy of this form to give to the office as it should list all the students who have signed up for the sack lunches. *
