We need to know if you are NOT planning to attend Rice Lake Elementary for the 2022-2023 school year. This is for families of current K-4 grade students only. If one of the following applies to your family, please complete this form.
1. families who are not moving but whose children will attend a different school (do not include 5th graders)
2. families moving and will go to another school within the District
3. families moving out of the District
4. families moving out of the State or Country

Each family only needs to complete one form. Start with your youngest student first and add additional student information in sections that follow.
