T1D - Patient Perspective
Hello, My name is Brooke. I am a 4-year Ph.D. student and my research involves developing a biomaterial intended to help patients with Type-1 Diabetes. 
Having witnessed my mother developing T1D later in life and seeing the complications she endures daily,  I've drafted this form to collect information from other T1D patients to help guide my research.
No personal information is collected through this form. The information provided will be used to guide my research decisions. 
If you feel comfortable sharing your experience either as a patient with T1D or a caregiver to someone who has T1D I would love to hear your perspective.
