I grant permission to the Camp director, assistants and or assigned chaperones of the Camp to act on my behalf for said Camper. I also grant permission for physicians representing the Camp to treat said Camper in case of health emergencies or illness, and hereby waive and release the Jamaica Basketball association from any liability for any injury or illness incurred while at the Camp. Expenses resulting from illness or injury, which involve emergency treatment, are the responsibility of the Campers or legal guardians.
