For Returning Families and those Transferring from Another CCSD School
After completing this form, go to campusportal.ccsd.net to complete online registration. Once you login a link will be available on the left panel of your Parent Portal for registration. If you have an incoming Kindergartener, click on the STUDENT tab to Add New Student.

To complete your registration, you must email the following to 0366-inbox@nv.ccsd.net (Please include your child's name and the application number in the subject line):
-Proof of Address (1-current utility bill (gas/water/elec)
-Parent Photo ID
For Families NEW to CCSD
Go to register.ccsd.net. Click on the OLR tile. To complete your registration, you must email the following to 0366-inbox@nv.ccsd.net (Please include your child's name and application number in the subject line):

- Student(s) Birth Certificate
- Immunization Records
- Parent Photo ID
- Proof of Address (1-current utility bill (gas/water/elec), new lease or escrow docs)
CCSD Student Health Information for School Year 21-22-All students must have health information on file. You can print the form below, scan the completed document, and email it to 0366-inbox@nv.ccsd.net. Additional copies of this form will be in the May Elementary Office.
Network Release-The form below will assist you in completing the Annual Release and Network Access Form.
