Request for a change in the name
This form may be used by currently registered students, identifying as gender variants, to officially request the College Administrations to make changes and/or amendments to the name of the student.  Fill in this form in a safe space so that you are free to be true to yourself.

Students requesting changes must be aware that there are conditions and limitations imposed by the law.
Once the request is submitted, the Vice-Principal shall hold an interview with the student requesting the change to identify ways of support and the way forward together.

The information given in Section 1 may be shared with administrators on a need-to-know basis.
